What is the Primary
Care Network?

The Chilliwack and Fraser Health Rural PCN brings together family physicians, nurse practitioners, and allied health professionals to deliver coordinated, team-based care. Built on a foundation of Patient Medical Homes, the PCN strengthens primary care through shared care plans, expanded services, and strong collaboration across disciplines.

This approach ensures patients receive care from the right provider at the right time. Family doctors and nurse practitioners lead primary care, while clinical counselors, dietitians, pharmacists, and other allied health professionals provide wrap-around support tailored to individual needs. Every team member works to the top of their scope, enhancing care while easing the load on providers.

For patients, the result is timely, culturally safe care and better connections to community services. For providers, it means more time with patients, a broader care team, and practical support to deliver high-quality, relationship-centered care.

The PCN serves residents in Chilliwack, Agassiz-Harrison, Hope, the Fraser Canyon, and 22 Indigenous communities from Chilliwack to Boothroyd, including Métis and Inuit families. Allied health services are available free of charge for eligible patients. Services include:

Clinical Counselling

A Clinical Counsellor offers short-term one-on-one counselling for individuals with mild to moderate mental health conditions.

Social Work

A Social Worker can assist in developing: coping strategies, problem-solving skills, self-care management and strategies for managing/navigating challenging situations.

Chronic Conditions Nursing

A Chronic Conditions Nurse supports the patient’s self-management of mild to moderate chronic conditions.

Dietitics

A Dietitian develops nutrition goals and plans that will guide patients to embrace food, enjoy it and support healthy living.

Physiotherapy

A Physiotherapist helps restore movement and function in patients affected by injury, illness or disability.

Occupational Therapy

Occupational Therapists aim to improve or maintain a client’s participation and engagement in activities of everyday life through a variety of interventions

Primary Care Clinical Pharmacy

A Primary Care Clinical Pharmacist focuses on preventing and resolving medication-related issues for patients with complex conditions or healthcare needs.

Registered Nurses in Practice

Through the Primary Care Network (PCN), Registered Nurses in Practice (RNiP) are co-located in family practice clinics to provide comprehensive support for patients. These RNs are employed by the Fraser Health Authority and work directly in your clinic, alongside family physicians and nurse practitioners to enhance continuity and capacity for patients.

By integrating into team-based care settings, RNiPs help patients feel more connected to their care journey, while giving physicians and NPs more time to focus on what they do best.
